Affiliate Application

I understand and agree that: Any deliberate misrepresentation or omission of facts in my application may be justification for refusal of, or if employed, termination from employment. It is my understanding that Mentor Services Unlimited, LLC (The Parent Company of the Six Figure Club) and/or any of the consortium companies represented by the parent company, including but not limited to: Six Figure Club may make a review of my history to verify all data given in my application for employment, related papers, or oral interviews. I authorize such review and the giving and receiving of any information requested by Mentor Services Unlimited, LLC and I release from liability any person giving or receiving any such information. I understand that inability to verify data or derogatory information discovered as a result of this review may prevent my being hired, or if hired, may subject me to dismissal. I agree that my employment may be terminated by this Company at any time without liability for wages or salary except such as may have been earned at the date of such termination. I authorize any physician or hospital to release information which may be necessary to determine my ability to perform the duties of a job I am being considered for prior to employment or in the future during my employment with Mentor Services Unlimited, LLC. Although Mentor Services Unlimited, LLC may make every effort to accommodate individual preferences, business needs may at times make the following conditions mandatory: overtime, a rotating work schedule, or a work schedule other than Monday through Friday. I understand and accept these as conditions of my continuing employment. I understand that this is an Application for Independent Contractor Employment, and that NO Employment Contract is being offered. I understand that, if I am employed, such employment is for no definite period of time and that Mentor Services Unlimited, LLC (The Parent Company can change wages, benefits, and conditions in the future). After review by our Board of Executive Directors, you will be notified if you were selected for the position you are making application for. This is an electronic application and is considered a legal document. Your completion of the below information is to be considered your application for a position within our Consortium of companies, whereas the parent company is: Mentor Services Unlimited, LLC.
